Care Coordinator/Registered Nurse (RN)

Position Summary

As a Care Coordinator/Registered Nurse (RN) at RewireInHope, you play a crucial role in providing high-quality, compassionate care to our clients. Registered Nurse will provide high-quality patient care, support health education initiatives, and collaborate with medical staff to ensure optimal patient outcomes. RN will play a crucial role in assessing patient needs, implementing care plans, and fostering a healing environment.

Essential Functions

Under the direction and supervision of the Program Coordinator, the Direct Support Professional is responsible for but not limited to:

  1. Patient Assessment: Conduct comprehensive assessments, including reviewing medical histories, performing physical exams, and measuring vital signs to determine patient care needs.

  2. Care Planning: Develop and implement individualized care plans grounded in patient assessments and continuous evaluations, collaborating with interdisciplinary teams to optimize patient outcomes.

  3. Medication Administration: Administer prescribed medications and treatments in accordance with established safety protocols, while monitoring for any adverse reactions and ensuring compliance.

  4. Patient Education: Provide thorough education to patients and their families about diagnoses, treatment plans, medications, and effective health management strategies to encourage self-care.

  5. Monitoring and Documentation: Consistently monitor patient progress, accurately document findings in medical records, and promptly report any significant changes in patient condition to the healthcare team.

  6. Infection Control: Implement and adhere to established infection control protocols and procedures to minimize risks to patients and staff.

  7. Emergency Care: Respond effectively to medical emergencies, applying appropriate interventions and assisting in critical care situations as required.

  8. Regulatory Compliance: Strictly adhere to local, state, and federal regulations governing nursing practice, ensuring the highest standards of care are consistently maintained.

  9. Professional Development: Engage in ongoing education and training to maintain licensure and remain informed about best practices in nursing.

  10. Collaboration: Collaborate effectively within a multidisciplinary team, fostering a positive and supportive work environment for all team members.

  11. Communication and Collaboration: Maintain open and effective communication with clients, their families, and all members of the healthcare team to ensure cohesive care delivery.

  12. Adherence to Policies and Procedures: Follow established policies and procedures to uphold the highest standards of care and ensure compliance with regulatory requirements, reporting any incidents or accidents as appropriate.

Requirements

  • Successful completion of a state-approved Registered Nurse program.

  • Current Registered Nurses license from state.

  • Valid Basic Life Support (BLS) certificate.

  • Valid Advanced Cardiovascular Life Support (ACLS) certificate.

  • Strong communication, organizational, and interpersonal skills.

  • Ability to handle multiple tasks and work in a fast-paced environment.

Pay Range: $40/hr - $45/hr
